Will Larger Breast Implants Create More Lower Pole Fullness?

Q: Dr. Eppley, I am inquiring about a breast implant revision. I currently have 800cc silicone over the muscle with galaflex. I would like more lower pole fullness and more volume. If possible, please let me know if this is an option as I know it’s not a typical revision.

A: Thank you for your inquiry and sending your pictures. I would agree with you that to improve the lower pole fullness you do need more volume ,probably going from 800 to at least 1000 -1100 cc. The key question in so doing is what is he pathway of entrance for the implant replacements and will that disrupt the Gala flex support that you now have. I am going to assume that you have inframammary incisions from your prior breast implant surgeries. Galaflex eventually over time becomes replaced by scar tissue so it is not like cutting through synthetic mesh. How long ago was the Galaflex mesh implanted?
