How Can I Make My Chin More Square After A Sliding Genioplasty?

Q: Dr. Eppley, here are some current photos of me now and then photos of me before I had my chin implant done as well as aesthetic goals as well. I’ve since had the chin implant removed after it cause fairly severe bone absorption and had a genioplasty to try and repair it but I found its not quite back to the sort of square chin and less rounded nature I have now. I think it looks good but I find it just bothers me still because of the rounded edges and because it isn’t square like how my chin used to be. 

A:Thank you for your inquiry and sending your pictures. While a sliding genioplasty can be a very effective chin augmentation procedure the one thing that it cannot do is square out the chin. This is a chin reshaping effect that only an implant can achieve. I have seen your exact situation numerous times and is treated with a custom square chin implant overlay with the greatest emphasis on bringing out the sides of the chin more than the center to create a more square chin look.
