How Can I Fix The Divots In My Jawline After A Sliding Genioplasty?

Q: Dr. Eppley, I have two divets in my jawline, one on each side, not too far from my jaw angle. These are from a mandibular osteotomy done in the 90s. Can these divets be filled in somehow? I have looked into dermal fillers online; the area seems to be a sensitive one so I’m a little apprehensive about that. I’m attaching a picture of my right jawline. Unfortunately, you can barely see the divet here, but it really impacts the smooth line of my jaw. I’m a transitioning transwoman and I’d like to have a nice smooth jawline. Thank you

A: These are classic jaw angle/antegonial notch deformities from mandibular osteotomy surgery. They are best reconstructed by an implant which is ideally done using a custom design approach since they are undoubtably asymmetric in shape. Short of this ideal approach there are standard vertical lengthening jaw angle implants which can be used but applying standard shape implants to a noin-standard shape problems has its own disadvantages.
