How Can I Bring My Chin and Jaw Down and Forward?

Q: Dr. Eppley, Hello I am interested in a chin or jaw procedure to bring my chin out and jaw down so my lower face doesn’t look so short, also reduce the deep of the labiomental fold.

A:With a vertically chin the mandibular plane angle is flat the labiomental fold is deepened and the lower lip rolls outward. Certainly you need the chin vertically lengthened and that is in the 10mm range which means it has to be done by a vertically lengthening bony genioplasty. While the jawline behind the chin can be simultaneously augmented, and a debate can be had about whether it needs any vertical lengthening at the jaw angles, this has to be done by a custom jawline implant that merges into and wraps around the bony genioplasty for a total jawline augmentation effect.
