Can V Line Reversal Be Done With Bone Grafting?

Q: Dr. Eppley, I am interested in undergoing reversal V-line surgery and would like to know the best approach for achieving a natural-looking jawline while still being able to participate in activities such as sparring in the future. I aware that custom jawline surgery is a common option, but I am curious if custom bone grafting would be a more natural and stronger option.

A: Onlay bone grafting is never a viable option for any form of aesthetic jaw augmentation for a number of  practical and biologic reasons, most notably the high and irregular absorption of the bone grafts that will occur.
