The use of injectable collagen solutions for the filling out of depressed facial scars, fine lines and wrinkles, and enhancement of other facial structures (e.g., lips) has been successfully done for the past twenty five years. While they work well for their intended purposes, collagen-derived have always been plagued by quickly being absorbed and going away. Plus, some people were allergic to these animal-derived collagen materials.

Over the past 10 years, many new and differently formulated injectable fillers have become available. Today hyaluronic acid, a natural sugar, has essentially replaced collagen as the preferred injectable material. Numerous formulations of hyaluronic acid exist (e.g. Restylane, Juvederm, Perlane) and are widely used around the world. They are longer lasting (4 to 8 months) and are associated with fewer risks of sensitivity to the material. Most recently, a new collagen filler (Evolence) has emerged that lasts longer than 6 months and is not associated with the previous problems of older collagen materials. Also, a particulated filler (Radiesse) is another option which offers longer lasting effects (9 to 12 months) due to the slowly resorbing ceramic particles that it contains. Because of the risk of clumping or irregularities, it is not used in the lips but can be used deep at the bone level for temporary cheek, chin and jaw enhancement.

Due of the number of commercially-available injectable fillers, the best choice for any patient and their concerns should be discussed with a plastic surgeon. Choosing the best injectable filler is based on understanding where they can be placed, how long they will last, and how much they will cost.
